Where is the rental property?

Which state is the rental property in?

The notice itself does not name a governing law. The state selects the notice rules LeaseCraft has recorded, which are shown beside your document as a checklist and can stop a notice the state does not allow.

What is the address of the rental property?

Include the unit number, city, state, and ZIP code. This is the address the notice is about.

What kind of notice?

Choose the notice that matches your reason. The wording of the notice and the state rules that apply follow from this choice.

Which notice do you need?

Rent is late: choose "Pay rent or move out". The tenant broke a lease term that can be corrected (an unauthorized pet, a parking rule): choose "Fix a lease violation or move out". Serious or repeated violations, illegal activity or damage, where you are not offering a chance to fix it: choose "Move out — no chance to fix". Ending a month-to-month or week-to-week tenancy without stating a cause, or not renewing: choose "End a tenancy without stating a cause". The lease has ended and the tenant has stayed: choose "The lease has ended".

What kind of tenancy is it?

A fixed-term lease has an end date. A month-to-month or week-to-week tenancy renews each period until one side ends it.

When did the tenant move in, or when did the lease begin?

Use the earliest date the tenant has lived at the property under this tenancy. Some states set longer notice periods for longer occupancies.

Yes No

For example, a spouse or roommate who signed the lease. Every adult tenant named on the lease should receive the notice.

The second tenant

Dates and details of the notice

Date on the notice

Date the notice is (or was) delivered to the tenant

Usually the same day as the notice date. The notice period is counted from this date, so enter the day you will actually deliver it.

Deadline for the tenant to pay, correct the problem, or move out

The last day the tenant has to act. Count it from the delivery date. The state checklist gives the minimum notice period LeaseCraft has recorded for your state and this kind of notice; a deadline that is too soon after delivery is stopped before the document is prepared.

Delivery

How will this notice be delivered?

The state checklist lists the delivery methods LeaseCraft has recorded for your state. Record what you actually did on the Certificate of Service.

Contact for questions about this notice optional

Leave blank to use the landlord's details above.

Documents attached to or served with this notice optional

For example, a rent ledger, photographs, or a form your state requires to accompany the notice.
