General-purpose template. Not reviewed for your state's specific requirements.

Step 1 of 65 sections · answer each, then create your document

Landlord

Full legal name of the person or business that owns or manages the property.
Use the address where rent payments and legal notices should be sent.

Tenant

Full legal name of the primary adult tenant who will sign.
Street address of the rental unit (not a P.O. box).

Move-out date

The date printed on this notice.
Deadline to leave if the tenant does not cure or if this is an unconditional quit.
The move-out date must be at least this many days after the notice date. It must also meet your lease and state law.
For a lease violation, use the Eviction Notice: it gives the tenant the chance to fix the problem that the law requires.
Some states, such as New York, require longer notice for longer tenancies.

Delivery

Use a method allowed by your lease and state service rules.
Choose Yes to remind the tenant to provide a forwarding address for deposit return.

State

Choose the state where the rental property is located.
