Step 1 of 1110 sections · answer each, then create your document

The photo booth provider

For a business, use its registered name, including "LLC" or "Inc." if it has one.
Used for written notices under the agreement.

The client

For a business, use its registered name, including "LLC" or "Inc." if it has one.
Used for written notices under the agreement.

The event

For example "The Old Mill Barn" or "Grand Ballroom, Hotel Meridian."
Street, city, state, and ZIP code, for example "410 River Road, Asheville, NC 28801."
The person the attendant can find at the event, with a mobile number, for example "Jordan Lee, 555-0142." Often a planner or a member of the wedding party rather than the client.

The booth and rental hours

For example "7:00 p.m."
For example "11:00 p.m."
The hours between the opening and closing times, not counting any idle hours entered below.
Enter 0 if the booth runs without a break. Idle hours are usually charged at a lower rate than open hours.
Charged per hour or part of an hour, and only if the provider can stay.
Setup time is not charged and is not part of the rental hours. 60 to 90 minutes is common.
Charged only if the client asks for the booth to be in place well before it opens, for example before guests arrive for a ceremony. Leave blank if early setup is not offered or is priced case by case.
"Yes" means the provider staffs the booth throughout the rental hours. "No" means the booth runs on its own and the client supervises it.

Space, power, and internet at the venue

For example "10 by 10 feet" or "8 by 8 feet with 9-foot ceilings."

Package, design, and deliverables

Choose everything the price covers. Anything not chosen is not part of the contract.
Must be fewer days than the deadline above, so the proof can use what the client sent.
"No" means the provider keeps the images private and shares them only with the client and guests.

Fees and payment

The price for the booth, the rental hours, and everything chosen as included. Idle hours, travel, and add-ons are entered separately and added to it.
For example an extra hour of props, a second backdrop, or a scrapbook upgrade. Leave blank if there are none.
The retainer reserves the date, is paid when the contract is signed, and is not refunded if the client cancels.
Enter 0 if the balance is paid on the day, before the booth opens.

Cancellation and rescheduling

A cancellation earlier than this forfeits only the retainer.
Enter 100 if the full balance is owed.

Damage, conduct, and insurance

Many venues ask for a certificate of insurance before an outside vendor can set up.
For example a parking arrangement, a vendor meal for the attendant, or the venue's own vendor rules.

Governing law

Usually the state where the event takes place.
